Background Information:
As mental health nurses we play a significant role in improving the physical health of consumers living with a mental illness. Several evidence-based tools are available to help guide the delivery of physical health care and ultimately achieve good consumer physical health outcomes. For this assignment you are required to identify a consumer who has or who is at risk of developing a significant physical health issue. You should provide a comprehensive physical health care plan for this consumer. Working with the consumer write up their background and case history. You should then utilise the Health Improvement Profile (HIP) tool with the consumer to identify areas of physical health need. What evidence-based interventions would you utilise with the consumer to improve their health as part of a health care plan. What changes to local practice would need to occur to improve outcomes for this consumer and how would you evaluate the strategy.

Read the following instructions carefully in order to complete this part of the assessment for this course.

1. Provide an introduction to your paper with clear objectives. You should also provide a succinct background and case history on the consumer you will be presenting in your report and why they were chosen. 

2. Using the Health Improvement Profile (HIP) tool available on Canvas, provide results of the comprehensive assessment for the chosen consumer. Please be aware that the HIP has several domains so your focus should be on those domains that are relevant to your chosen consumer. Please do not include the HIP form in your report, rather use subheadings to present your findings. 

3. The HIP provides pointers for the interventions to be put in place depending on the consumers result parameters. Using the suggested interventions as a guide, provide evidencebased intervention you would put in place for this particular consumer. Your intervention must draw on the existing literature and the particular area you are addressing, this can include established clinical practice guidelines. The interventions must also be consumer/ recovery focused. Consider what changes would need to be put into place in your organisation to ensure monitoring occurred and was reported and acted upon. 

4. It is important that interventions are evaluated for their effectiveness. Outline how you would evaluate the effectiveness of the suggested interventions including any strategies to enable the consumer to maintain a good physical health outcome by minimising or preventing deterioration in physical health. 

5. Provide a conclusion to the paper which highlights keys areas addressed as well as any learnings in relation to formulating a consumer physical health care plan in mental health settings. 

6. You should show evidence of research into the topic including reading beyond the required basic literature set for this course. References used must be relevant to the topic and must be contemporary (within the last 10 years). The types of references used can include textbooks, journal articles, professional organisations, and government policies. Students should research the topic through the RMIT Library databases and e?journals.
