Assignment Task

Task: Consider a patient you have assessed during your clinical placement. Use either the VINDICATED MEN or VITAMINS ABCDEK framework to analyse either a presenting symptom (eg abdominal pain) or a diagnosed condition (eg renal failure). The use the scales of probability to refine your differentials by examining the likelihood of each. Narrow your differentials down to a maximum of three and explain why these are the most likely diagnoses and what tests or additional information you require to confirm or exclude them.

General notes: This is an academic assignment and needs to be written as such. You must use academic prose and not bullet points or abbreviations. You must extensively reference relevant literature to support your discussion and arguments. Your clinical and NP mentor will be invaluable resources in completing this assignment. Structure your assignment as follows (PLEASE use these headings):

Introduction:

• Identify which framework you are using

• Give a brief summary of the patient being discussed and include the clinical setting in which you have encountered them

• Identify the symptom or condition you are discussing, and outline the history and examination findings from your assessment

• Briefly discuss the benefits of using a framework such as VINDICATED MEN or VITAMINS ABCDEK

Discussion:

• Work through your chosen framework, outlining for each item the characteristics that support or refute this differential. Include reference to the pathophysiology of each condition, as well as prevalence if available.

• You may have more than one differential for each section.

• Analyse the evidence (both from reference sources and from your assessment of the patient) in support of and refuting of each diagnosis, and determine how likely each is (low, medium, or high probability)
