Instructions To Complete The Task:

Context
You are an occupational therapist on an acute hospital ward where Frank Fontana, your client, is currently an inpatient. Frank was admitted to hospital following a fall at home. You have completed an initial assessment with Frank (see client notes and accompanying video of session with Frank). The medical team wants to discharge Frank as soon as possible. Frank’s goal is to be discharged home to continue to live independently. In order to support Frank’s goal, you need to collect some more information: you have conducted a home environment assessment (see video), and now need to identify Frank’s person and environment barriers and enablers to performing his occupations. You need to present to the team an assessment of Frank’s occupational performance, and outline what your role with Frank will be able to enable him to be safely discharged home to live independently and engage in his valued occupations.

Your task is to create a client ‘narrative’ based on the CMOP-E that addresses the above. It is a narrative in the sense that you forming a ‘story’ or narrative about your client and their experiences.

• This narrative is still formal academic writing with use of cited research, but the emphasis is on applying information in order to analyse, consider, hypothesise impact on a client – as opposed to recalling facts about conditions or classifying/describing concepts (which was more the emphasis in your Health Condition Report)
• You may write the report either in first person narrative (‘As the OT, I would…’) or in third person narrative (‘The role of the OT working with Frank would include…’). Whatever you decide, you need to still use formal academic writing.
