Students are required to complete three clinical logs using the template provided on page 11 of your clinical assessment document (CAD). 

One of the logs should include

• A Clinical Management Plan and
• The other two should each include accurate completion of an FP10 or Inpatient 

Prescription. 
It is recognised that as a prescriber there may be circumstances when a CMP is required; so one CMP must be completed. Students will not be penalized where the CMP covers a situation that can be adequately managed as an independent prescriber. The CMP should be presented as part of a clinical log A clinical log should record a client’s assessment data obtained during a consultation and the resultant action plan.

The log should demonstrate

• history taking,
• documentation,
•  decision making and
• Client management.

The clinical log should demonstrate how the working diagnosis was formulated and the evidence-base for client management.
Where a prescribing decision is made, the log should indicate why a particular drug was prescribed. For example if an antibiotic is indicated for an infection why Metronidazole was prescribed in preference to Erythromycin. Client confidentiality should be maintained at all times.

Reflection Essay

The student is required to undertake a critical reflection based on:

• One of the submitted clinical logs and
• Ensure it encompasses the criteria as per RPS Competency 7,8,9,10.
• Students should use a published reflective tool e.g. Gibbs (1988) or Johns (2000) to appraise one aspect of their prescribing practice and to make recommendations for service improvement
