(2020) explored this communication specifically in the acute care settings. To respond to this question, the authors used an integrative review based on seven international databases using a framework analysis approach. The first theme explored in the analysis is that patients and families should always be partners in decision-making and in the provision of care. Secondly, communication between families and the healthcare providers should be adhered to fully as it is the basis for provision of quality care services. Another notable observation was that lack of communication often results in difficulties in allowing families to take an active role in guaranteeing patients with much needed care and lastly, reported experiences of patients and care givers should form the basis for future engagement in the healthcare sector. The emergence of COVID-19 has had remarkable implications in the manner in which healthcare services are given to patients. The pandemic has necessitated changes to how services are given to patients with pharmacy transition services being part of such changes. According to Herzik and Bethishou (2021), the COVID-19 management measures like minimized contact with the patient and altered patient visiting schedules has made it a challenge to institute traditional medication practices. The authors content that it is now challenging to get the list of the medications the patient was using prior to visiting the facility. Consequently, people rely on secondary sources to have an idea of the medications the patient was using. In discharge procedures, focus has shifted from use of hard copy prescriptions to electronic prescriptions and use of other medication programs that can reduce the contact between the patient and the provider especially in the outpatient facilities. Older patients fall among the most vulnerable groups when it comes to COVID-19. Besides most of them having underlying conditions, the elderly patients also have a reduced immunity and hence have to be closely monitored so as not to succumb to the condition. In this study, Ozavci et al. (2020) looks at the challenge of communication in managing medications especially in older people who move across the transition of care. According to the authors, any slight breakdown in communication can lead to emerging incidents or discrepancies in medication. The study employed a comprehensive systematic review of both qualitative, quantitative and mixed method from databases such as CINAHL, MEDLINE and Embase. The inclusion criteria used by the authors included studies about medication management perspective for people aged above 65 years and who experienced a transition of care. Key article cross-checking and hand searching of reference list was also done.

After a final review of 33 articles, the authors noted that poor communication between primary and secondary care settings was the main reason for existing medication discrepancies that older patients were subjected to before discharge. According to the authors, older patients expected a communication approach that is ongoing, timely, accurate and tailored to address their specific needs. Furthermore, the written information regarding their medication has to be clearly done to help improve the post discharge process. In their conclusions, the authors argued that communications regarding medications was often ineffective with little consideration of the older patient’s views about communication during admission, hospitalization and in transfer.

The authors discussed the importance of using quality of life (QoL) as the basic outcome in evaluating the transition of care interventions in seeking to improve the health of patients that have been hospitalized with acute conditions. According to the author the existing transition of care interventions may not be effective for patients of all races and ethnicities and are also not conscious of the age. The systematic review methodology employed in responding to this research question was appropriate since it gave a wider insight into the intervention strategies employed by other scholars.

Quality of life can be determined better when monitoring patients with chronic illnesses. Therefore, for the study, the population of focus was patients with cardiovascular conditions, chronic obstructive pulmonary disease, kidney failure, breast cancer and stroke. After a review of the articles, seven Quality of Life instruments were identified with most of them being multifaceted in nature with 31 strategies. Some of the QoL strategies discussed in the article include having an intervention facilitator raining and the use of intervention materials. From the evaluation, it was evident that most scholars omit the cultural and age aspects when designing the interventions. Consequently, it is important that future research explores other dimensions as a way of reducing future health disparities.
