The Registered Nurse (RN) providing care to Alex has conducted a respiratory assessment and formed a clinical judgement that there is inadequate oxygen entering the blood in the lungs. Based on their understanding of the causes and mechanisms of cell injury, recent assessment findings, and Alex, medical history, the RN has promptly commenced supplemental oxygen, to maintain adequate oxygenation
